چکیده

Introduction Shunt-induced hypoxemia is considered the primary pathophysiological abnormality and main diagnostic criteria of acute respiratory distress syndrome (ARDS). However, increases in dead-space ventilation (VD/VT) can also contribute to gas exchange alterations in ARDS. Systemic microcirculatory alterations described during inflammatory conditions are characterized by perfusion heterogeneity and theoretically pulmonary microcirculatory heterogeneity could lead to imbalance pulmonary ventilation/perfusion relationship. Thus, we hypothesized that systemic microvascular alterations could reflect increased VD/VT in ARDS.

Quantifying the severity of acute lung injury using dead-space ventilation: should the lung injury score be updated?

During the past decade of research on acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) there has been heightened interest in measuring physiologic dead-space fraction (VD/VT). Since ARDS first was described 44 years ago, the primary focus on gas exchange abnormalities concerned oxygenation. In fact, until the publication in 2002 by Nuckton et al,1 pulmonary dead-space vent...

Prognostic value of the pulmonary dead-space fraction during the early and intermediate phases of acute respiratory distress syndrome.

BACKGROUND Little is known about the alveolar dead-space fraction after the first week of acute respiratory distress syndrome (ARDS). We measured the dead-space fraction in the early phase (first week) and the intermediate phase (second week) of ARDS, and evaluated the association of dead-space fraction with mortality. METHODS We prospectively measured dead-space fraction and other variables ...

Effect of bronchodilators on lung mechanics in the acute respiratory distress syndrome (ARDS).

The acute respiratory distress syndrome (ARDS) is a disorder of diffuse lung injury secondary to a wide variety of clinical insults (eg, sepsis) and is manifested by impaired oxygenation, pulmonary edema, and decreased static and dynamic compliance. More recently, airflow resistance has been shown to be increased in humans with ARDS. We designed a prospective, randomized, placebo-controlled, cr...

Potential effects of corticosteroids on physiological dead-space fraction in acute respiratory distress syndrome.

BACKGROUND Increased dead-space fraction is common in patients with persistent acute respiratory distress syndrome (ARDS). We evaluated the changes in the oxygenation and dead-space fraction in patients with persistent ARDS after corticosteroid therapy. METHODS This was a non-randomized non-placebo, controlled observational study including 19 patients with persistent ARDS treated with cortic...

The association between physiologic dead-space fraction and mortality in subjects with ARDS enrolled in a prospective multi-center clinical trial.

BACKGROUND We tested the association between pulmonary dead-space fraction (ratio of dead space to tidal volume [V(D)/V(T)]) and mortality in subjects with ARDS (Berlin definition, P(aO2)/F(IO2) ≤ 300 mm Hg; PEEP ≥ 5 cm H2O) enrolled into a clinical trial incorporating lung-protective ventilation. METHODS We conducted a prospective, multi-center study at medical-surgical ICUs in the United St...
